I wouldn't worry too much about the bounce rate - especially as I don't see any other navigation on the landing page. Where else are they going to go except to a conversion page?
I don't see any issues with the design.
It does look really thin on content though. There isn't much compelling encouraging someone to call or fill out the form. You're targeting knees and mention that he's a shoulder specialist. I know it is difficult, but getting some testimonials - written with photos are good - video if you can get it would be huge. A story of someone - maybe a runner - who was injured, but the doc got him back into action - with before and after photos.
Also, people don't want an evaluation, they wan't their knee fixed. While you could offer the evaluation, I would also talk about how he's been able to get people with various knee injuries back to living an active lifestyle.
