I filled it out, but the way that the questions are phrased, and the choices that are given are going to result in data that is not particularly insightful. For example, the question about getting an email from the city about water supply: wouldn't the correct choice be between .com vs. gov? Also, the question about reliable shipping leaves it open to location bias.
Just a couple of thoughts. I would refer you to Stephanie Beadell's excellent presentation on this subject from Mozcon 2014 (which I believe is behind a paywall.

I can't speak to the last day of the month, but I know that I load up on purchases towards the end of the year because I know that I can deduct business related expenses, and want to cram them into the current fiscal year.

I'm curious about the types of programs that you have used in the past to surface comments that are not appropriate for a given website. Did you ever see false positives in peoples' phrasing in comments, such as (and this is a terrible example), someone using the word Dick, but to address a person rather than to call someone a name? Did you have to do this manually, or has someone created a better filtration system than this?

How do you guys feel about the new design and layout of the Inbound.org? I'm generally resistant to change so I'm not crazy about it. The previous version felt more intuitive to me. Perhaps Ed Fry can give some insight into why the change was made?
